Belize Visa Requirements

• 1 Application Forms, fully completed and signed
• 1 Passport-type photograph
• Completed Cover Page and Credit Card Authorization forms (print from browser)
• Submit your valid passport, must have 6 months remaining validity
• Evidence of legal residence in U.S.A. (ie: valid multiple entry visa with I-94 attached, copy of alien registration card)
• Notarized copy of green card is required
• Copy of return/onward airline tickets or itinerary. Itinerary should show specific entries to Belize when requesting a Multiple Entries visa.
