1 Passport Requirements: Original, signed passport valid for 6 months beyond stay, with at least one blank visa page available for visa stamp(s). Amendment pages in the back of the passport are not suitable for visa stamps. 2 Azerbaijani Visa Application Form: One visa application form per applicant properly completed and signed. Note: the application must be typed, handwritten applications will not be accepted. Signature must be original on each application. Improperly completed forms may delay the processing of your visa. 3 Photo Requirements: Two recent 2 x 2 passport type photographs, in color, front view and with a plain/light background. 4 Official Invitation from Azerbaijan: Confirmation from a travel organization or hotel in Azerbaijan or from a private citizen of Azerbaijan must be registered through the Consular Department of the Ministry of Foreign Affairs of Azerbaijan. For a private visa, the applicant must provide the exact address and phone number of the inviting party and submit a copy of the Azerbaijani citizen identity document. 5 Proof Of Departure: Computer generated flight itinerary from the airline or a travel agency, copy of airline ticket or a corporate letter stating travel plans. A signed letter on the travel agency’s letterhead confirming round-trip booking may also be submitted. 6 Special Instructions: All foreigners visiting Azerbaijan for a period of more than 30 days must register their passports with the local passport-registration authorities. 7 Global Reservation Form: This is Travisa’s service order form, where you will select the visa processing needed and make your payment.
