1 Passport Requirements: Original, signed passport valid for 6 months beyond stay, with at least one blank visa page available for visa stamp(s). Amendment pages in the back of the passport are not suitable for visa stamps. 2 Armenian Visa Application Form: One visa application form per applicant properly completed and signed. Signature must be original on each application. Improperly completed forms may delay the processing of your visa. 3 Photo Requirements: One recent 2 x 2 passport type photograph in color, front view and with a plain/light background. 4 Proof Of Departure: Computer generated flight itinerary from the airline or travel agency or copy of airline tickets. 5 Other Requirements: If you plan to stay in Armenia for a period longer than 120 days, you must apply for a Temporary Residence Card at the Passport and Visa Department of the Police Service of Armenia (Address – 13 A Mashtots Street, Yerevan, Armenia; Tel. (3741) 53-43-91 or (3741)52-14-16. 6 Special Instructions: Please note that the Embassy of Armenia in Washington, DC is closed on Wednesdays. Please take this into consideration when sending your documents to Travisa for processing. 7 Global Reservation Form: This is Travisa’s service order form, where you will select the visa processing needed and make your payment.
